How to Choose the Right AHU for Your Hospital or OT.

When hospital owners plan a new OT, they spend weeks comparing lights, tables and pendants. The AHU usually gets chosen at the end, in a hurry, when the budget is almost gone.

That is a risky order. The air handling unit decides whether your theatre holds its temperature, passes air-quality checks and stays safe for patients. A wrong choice shows up later as high power bills, failed tests and delays in approval, and by then fixing it is expensive.

This guide covers what to look for, what an AHU roughly costs in India, and why it should be planned together with your modular operation theatre.

What an AHU actually does in an OT:

An air handling unit (AHU) filters, cools, dehumidifies and moves the air in a building. In an operation theatre it also keeps the room at positive pressure, so clean air flows out towards the corridor and dust or germs can't drift in.

A split AC only recirculates room air. It has no hospital-grade filtration and no pressure control, so it can never stand in for a proper AHU for operation theatre use.

5 things to check before you buy:

1. Give every OT its own AHU

Sharing one unit between two theatres saves money on paper and creates cross-contamination risk in practice. A dedicated AHU also lets each OT run on its own schedule, which saves power on days when one theatre is idle.

2. Get the capacity calculated, not guessed

AHU size is measured in CFM (cubic feet per minute). It depends on your OT's size and ceiling height, how many times per hour the air must be changed (operation theatres commonly need 20 or more), the heat from lights, equipment and staff, and your local climate. Delhi NCR in June is a very different load from Bengaluru in January.

Too small and the unit can't hold temperature or pressure. Too large and you pay for electricity you don't need, with humidity problems on top. Ask your HVAC consultant for the calculation instead of accepting a thumb rule.

3. Look at the full filtration chain

A good OT AHU cleans air in three stages: a pre-filter for larger dust, a fine filter that protects the final stage, and a terminal HEPA filter that removes extremely small particles, including most bacteria. Ask for the grade and efficiency rating of each, and confirm the HEPA sits at the point where air enters the OT.

For air-cleanliness classes, projects often refer to ISO 14644-1. It classifies air by particle concentration, but it is not a complete OT design specification on its own.

4. Inspect the build, not just the brochure

Two AHUs can look identical and behave very differently. Check for double-skin insulated panels, airtight joints, a sloped drain pan that is easy to clean, efficient fans, and filter doors you can open without a ladder and an hour of work. If filters are hard to reach, they won't get changed on time.

5. Plan it together with your modular OT

This is where projects go wrong most often. The AHU comes from one vendor, the OT from another, and the ducting, grilles and ceiling don't line up. Supply and return positions, HEPA diffusers, door design and sealed wall and ceiling panels all affect how air moves through the room.

At MedGenz, one of the established modular operation theatre manufacturers in India, we plan the OT and its air-handling needs as one project, so the room and the air work together. You can also review your layout early in our 3D OT design studio before anything is built.

AHU price in India: a realistic range:

There's no single price, because cost depends on capacity, filtration, controls and build quality. The ranges below are rough, for budgeting only, and are not a quotation.

AHU typeTypical useIndicative price (INR)
Basic hospital AHU (pre + fine filters)Wards, OPD, general areas₹2.5 lakh to ₹6 lakh
Standard OT AHU with HEPAMinor and general OT₹5 lakh to ₹10 lakh
Advanced OT AHU (HEPA, dehumidification, automation)Ortho, neuro, cardiac OT₹9 lakh to ₹18 lakh
Cleanroom-grade AHUIVF lab, transplant, critical cleanrooms₹12 lakh to ₹25 lakh and above

Add-ons that move the price

Add-onWhat it doesApprox. extra cost
Extra HEPA stagesHigher air cleanliness₹50,000 to ₹2 lakh
Humidity control (dehumidifier, reheat coil)Stable humidity in humid cities₹1 lakh to ₹3 lakh
VFD (variable frequency drive)Lower power use, flexible airflow₹40,000 to ₹1.5 lakh
BMS integrationRemote monitoring of temperature, pressure and filters₹1 lakh to ₹4 lakh
Filter alarms and pressure gaugesWarns you when filters need changing₹20,000 to ₹80,000
Stainless steel internal finishBetter hygiene and durability10% to 25% on base cost

Note: The pricing details provided in the table above are for illustrative purposes only and may vary depending on specific vendors, manufacturers, and market fluctuations.

Installation, ducting and commissioning are quoted separately. Retrofitting an old building costs more than fitting a new one, and prices also shift with brand, city and how many theatres you're building.

Please don't pick on the lowest quote alone. A cheaper unit with weak filters and leaky panels usually costs more over time through power bills, breakdowns and failed air tests. Over ten years, electricity and maintenance can add up to more than the AHU itself, so ask every vendor about power consumption, filter replacement cost, spare-part availability in your city and service response time. A good AMC/CMC plan keeps filters fresh and the OT audit-ready.

5 mistakes hospitals make:

  • Sharing one AHU across several OTs, which raises cross-contamination risk.
  • Sizing by thumb rule instead of a proper load calculation.
  • Ignoring humidity, which hurts most in coastal and monsoon-heavy regions.
  • Buying the AHU and the OT separately, then finding the two don't fit.
  • Skipping filter monitoring, so clogged filters go unnoticed for months.

Compliance and accreditation:

If you're aiming for hospital accreditation, your air-handling design will be reviewed. NABH programmes are open to healthcare organisations of every size and ownership type, and infection control is a core part of the assessment. Bring in a qualified HVAC consultant early, follow current healthcare ventilation guidance from ISHRAE and ASHRAE, and keep records of air-quality tests and filter maintenance.

Common Questions

Article FAQs

What type of AHU is used in an operation theatre?

A dedicated, double-skin AHU with multi-stage filtration, including a terminal HEPA filter, plus control of temperature, humidity and pressure.

What is the cost of an AHU for an OT in India?

Roughly ₹5 lakh for a standard unit up to ₹18 lakh or more for an advanced one with automation and humidity control. The final figure depends on capacity, specification and site.

Can I use a normal AC in an operation theatre?

No. Regular ACs lack HEPA filtration, controlled fresh air and pressure control.

How often should AHU filters be changed?

It depends on usage and local air quality. Pre-filters need the most frequent attention and HEPA filters usually last longest. Pressure gauges show when a change is due, and your manufacturer and consultant should set the schedule.

Can MedGenz handle the AHU and the modular OT together?

Yes. We plan OT design, installation and air-handling requirements as one coordinated project. Talk to our team about your hospital.
